Appearance Request

The Utah Blaze handles player, coach, dancer and mascot appearance requests for charitable and community events. If you would like the Utah Blaze to make an appearance at your event, please fill out the form below!

*All appearance requests must be submitted at least four (4) weeks in advance of your event. Requests sent under that timeline may not be able to be fulfilled. Due to the numerous requests received each week, we are unable to fulfill every request.
